PEDIATRIC NURSE PRACTITIONER TO PEDIATRIC-ACUTE CARE NURSE PRACTITIONER FELLOWSHIP

Job Summary

The Duke Pediatric Nurse Practitioner (PNP) to Pediatric-Acute Care Nurse Practitioner (PNP-AC) Advanced Practice Provider (APP) Fellowship is designed to support Pediatric Nurse Practitioners with a current family or primary care certification gain acute care certification to practice in intensive care units or stepdown units at Duke Hospital. This role coincides with acute care educational training at the Duke School of Nursing or other recognized accredited pediatric acute care training institution while concurrently obtaining supervised acute care clinical hours on the wards. The educational and clinical hours prepare the fellow to obtain the appropriate requirements to be eligible for pediatric-acute care certification by the completion of the program.

Specific timelines for this particular example case includes:

Fall semester (8/2026 – 12/2026) = at least 1 didactic course with 1-2 days of clinical in a non-ICU setting

Spring semester (1/2027 – 5/2027) = at least 1 didactic course with minimum 112 supervised clinical hours in ICU setting

Summer semester (5/2027 – 8/2027) = at least 1 didactic course with minimum 160 supervised clinical hours in ICU setting

For this specific example, the PNP to PNP-AC fellow will undergo clinical hours with the Heart Center non-ICU services, Pediatric Intensive Care Unit (PICU), and Pediatric Cardiac Intensive Care Unit (PCICU) with the goal of transitioning to the PCICU as a full time employee upon acute care credentialing.

The PNP to PNP-AC Fellow will work alongside APPs and MDs to provide direct patient care services for medically ill pediatric patients from birth to young adults at Duke University Hospital or in the clinics in collaboration with APPs, medical physicians, and surgical physicians.  The PNP to PNP-AC Fellow performs physical exams, collects and documents data, conducts diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, prescribes appropriate interventions and medications, coordinates the care and discharge of patients, and provides direct patient care services.  The PNP to PNP-AC Fellow contributes to excellence in patient care, research, teaching, and learns to provide leadership in the organization.

Goals of PNP to PNP-AC fellowship:

Acquire a deep and broad knowledge base of the common anomalies and disease processes for which patients are hospitalized, understanding the pathophysiology, various presenting

clinical presentations and disease prognosis

Develop a diverse differential diagnosis and respective diagnostic and treatment plan based upon the patient's presenting symptoms along with evidence-based and cost-effective medical care

Manage patients effectively throughout the whole hospital stay, including admission, daily rounds and continuing evaluation, and dismissal, while remaining cognizant of the patient's or families emotional, physical and spiritual needs

Learn practice management, including strategies in training APPs and APP students

Develop keen verbal skills to effectively communicate with colleagues, physicians, patients and

ancillary staff to ultimately become a key part of the inpatient health care delivery team

Job Duties and Responsibilities

Clinical Expert:

In collaboration with attendings, surgeons, APPs, residents, and fellows, round daily on patients to assess, diagnose, plan treatment, set priorities and realistic outcomes, and evaluate the effectiveness and cost efficiency of patient care.

Provide and coordinate clinical  care for service specific patients through established protocols and under the hospitalist supervising physician or back up physician who is either on site or available by pager.

Collaborate with the residents, fellows, attending other NP/PA colleagues. nurses, and patient resource managers to formulate treatment plans and monitor patient progress.

Perform procedures specific to the work environment after demonstrated competency and within the guidelines of scope of practice and credentialing protocols.

Assess patients for change in status and institute appropriate interventions.  Initiate emergency interventions in an effort to stabilize the patient until physician support arrives.
